Failure to contact us 12 hours before your scheduled appointment will incur a missed appointment fee to the full value of your appointment. New Patient - Adult For new patients only An Initial Consult will take 30 minutes and payment is expected at the time of visit. If you are unable to find a time that suits your schedule please contact us on (03) 425 7373 as we are usually able to find another option.
New Patient - Child 3-12 y/o For new patients only Dear Parent, This visit is for an Initial Consultation for your Child aged between 3 and 12 years of age. This visit will take 30 minutes. Please fill out the health form that is attached to your confirmation email. If you are unable to find a time that suits your schedule please contact us on (03) 425 7373 as we are usually able to find another option.
New Patient - Infant 0-2 y/o For new patients only Dear Parent, This visit is for an Initial Consultation for your Infant aged between 0 and 2 years of age. This visit will take 30 minutes.
